What information do I need to bring with me to my appointment with Attorney William Grover Arnett?

To expedite the process please bring the following:

1.  Names, addresses and telephone numbers of all doctors who you have treated with at least in the last 10 years.  (This includes physical and mental doctors)
2.  Names and addresses of all hospitals that you have been a patient at whether at the ER, inpatient or outpatient.
3.  All medical records and reports that you already have in your possession from doctors, hospitals etc.  If you don't already have any of these items, don't worry about that because it is our job to request and round up all  your treatment records.
4.  Bring any and denials that you have received from the Social Security Administration.  This information is essential because it allows us to know if you have the sufficient amount of time to appeal.
